The old (pre-circa 1985) Tomlinson's (the Greeks) in Bridgeport had similar rules to the Merritt, but they wern't posted, and you followed those rules not to expedite your order, but rather to avoid being thrown out the door and banned. When the guy behind the counter said "next" a type of stage fright would overwhelm the customer, and he'd better know his order, lest he lose his place in line, and his dignity.

There was a place in Richmond VA. In the Cary st area named Christies Cafe , Sadly long gone , There policy was no pennies. They rounded up or down,

My husband reminded me of another funny place we've been--Bob's Grill in Kill Devil Hills, NC--he has signs everywhere that say "Eat and get the hell out!" The wait staff encourages you to be on your way, with tongue in cheek, of course. The place is usually packed and they serve the biggest pancakes we've ever seen, so if your order them, it's hard to be as quick about eating and getting the hell out as Bob would like.
