What should be in a menu?

Hi! As restaurant industry pros, maybe you can help me out here. What would you normally want to see or show on a menu? For instance, I've read at some places that at the more chi-chi restaurants they don't put too many photos in their menus and instead just put descriptions. What do you think?

Also how should one go about listing the items and prices? Is there a more effective way to list them that sells more food?
 
Would appreciate any tips.
