IMHO, and I realize everyone like their burger many many different ways. I have worked in the food business for many years, I learned along time ago, I was doing my customer an injustice by telling them how something should be ordered and consumed. The day l removed the halo, thinking It was up to me to tell people how they should eat, or how their food should be prepared. When I designed menus, I always designed the recipes to meet the requirements and tastes of the majority of my customers. As a Chef, it was up to me to use the right amount of seasonings, not to over power,but, to blend for the taste I was looking for, for my customers.
   I watch some of the food shows, I see burgers that are twice the size of my mouth, sometimes even larger. My feeling is, the building of a perfect burger, should be, that, with every bite, a person should get a taste of everything on the burger. A burger should not me eaten with a knife and fork, a Burger should be eaten by picking it up and taking a bit.
The building of the Perfect burger........
1: The bun s/b made so it does not fall apart when eating, should be able to carry the weight of everything on it.
 
2: Lettuce s/b crisp I feel ice burg is the best, I think most restaurants use green leaf lettuce because it looks   better, and they are able to lay out a try of burger set-ups for faster service. I like the crunch of fresh ice burg lettuce.
 
3: The tomato s/b firm and full of flavor, I shake my head when I eat tomatoes in the winter. The tomato should not be to juicy so it drips and makes the burger a watery mess.
 
4: Bacon, s/b full of flavor, not greasy, not over or under cooked. A nice pepper bacon going real nice on a burger.
 
5: Mushrooms, fresh, sauteed in butter, not over or under cooked.
 
6: I put raw onion under the cheese just before I meal the cheese on the burger. I always have a hard tome keeping a onion slice from sliding off the burger.
 
7: there are many choices of cheese, any soft melting cheese works, just make sure its melted and running over the side a bit.
 
8: The burger, The burger s/b a good blend of meat to fat ratio. I want my burger cooked about Medium, still a bit pink in the center, full of juices. I feel a burger has to cook so the fats melt and combine to give the true flavor of the burger. There are many blends that people like. I use a 26 day dry age on my cows making the burger into a 85/15 blend, this works for me.
 
9: I use Mayo on the top bun, I put ketchup on as I go, I feel dips and spreads are different for everyone.
 
This is my version of a good burger, a good burger just doesn't happen, it's planned. this version is what I had on had in the house. If I were building the perfect burger I would not use a Store bought mass market bun. I would build a bun that would stand up to the weight of the burger & toppings.
 
 This in my opinion of a good quality, homemade, Bacon Mushroom Cheese Burger........
 
I let the burger rest for a few minutes, I want the juices to stay in the burger. As you see, you can cook a medium burger without cooking the hell out of it.
 

 
This burger just fit in my mouth ( I have a big mouth) I was able to get all the toppings with every bit. The burger is soft and silky, jicy, well seasoned with salt and pepper and melts in your mouth.

That looks fantastic Bill. That said, I can down a burger rare, med rare, med well, or well done. I love good burgers almost as much as bacon!!!
One thing to add in making a perfect burger, my staff MUST only turn the burger once, if I ever catch a cook pressing down on a burger with the spatula, they are gone!
What kind of cheese is that?

I'm hoping on my Segway and I'll be there in October.

 Mar, do you really have a Segway?  How do you like it?  My wife and I were considering a pair of them. but they are pricey -- for two, with some amenities, we were quoted about 15000.  I suspect there was negotiating room, but that's still pretty pricey for a toy.

I am thinking hard about the cheese and onion, though.  I always thought the cheese should be on top of the meat (gets more melty), and the onion on top of that.  With the cheese starting to soften a bit, it seems to hold the raw onion slices in place okay.  And I love a good, thick slice of a perfect raw onion.
 
I'll often add extra salt and pepper to the onion slices, just because that's what I do with a plain onion sandwich (bread, butter, onion, s&p).
 
But how about sautéed?  Sometimes a perfect burger is meat, cheese and fried onion, plain, no other condiments, maybe a toasted bun.

easydoesit, I agree on the onion, I like a think slice of a good onion. I'm 50 miles from Walla Walla so the onions are great. There are times a burger & cheese and a think slice of onion is great. I also love Caramelized onion, ...........My point was just what you said " Proportion" and enjoying each element in every bite.
 
Mar, I'll be waiting for you burger in hand, will you be pulling Sharon in a wagon behind the Segway...............

You are absolutely correct......  the onion should ALWAYS go on top of the cheese!
The good thing about burgers is that they can be prepared/cooked/grilled in SO many ways!
Burger topped with cheese....avocado, lettuce and onions.
Burgers grilled with sliced bits of Jalapenos sunk into them.
Burgers topped with Pineapple slices.
Burgers topped with bacon.
Burgers with 1000 Island dressing instead of Mayo.
There are 100s of ways to build a good Burger but ONE thing is for sure........NEVER put Ketchup on a Burger..........if you must use something........use BBQ sauce or Mustard!  I am also not a fan of using lettuce on Burgers either as it gets wilty and just doesnt do it justice! 
Chutney is also a VERY good topping! 
Dont forget the proven method of putting two Patties together with a BIG hunk of cheese between and grille em that way!

-6oz 80/20 coarsely ground mostly or all chuck seasoned with salt pepper and garlic, cooked on a very hot flat top to medium rare
-Caramelized onion
-Sharp american, two slices, one above and one below the patty
-Grilled sesame seeded bun
-Optional lettuce and tomato

"Never put ketchup on a Burger"?????....and why not? Please put the words, in my opinion, in front of that. Millions of Americans, I am sure, love ketchup on a good burger........As you say the great thing about burgers, like good pizza, is anything goes, if you like it , eat it!!

 this is the exact burger thats at the top of the page. It's a two handed burger, but, not a knife and fork burger. I wouldn't eat a knife burger,,,,,,,,,,,,

A Duane Purvis Burger? They're famous at the Triple XXX in Lafayette, Indiana. Not a fan, myself. I like their other burgers, but the Purvis is hugely popular.

Well, I don' like chocolate very much and certainly no vanilla swirl, I'm kind of in to fruit berry gelatos right now. But I still like my burgers fairly plain because I like a good meat flavor.

Preheat dry cast iron skillet to medium high. 
Form 80/20 beef in 1/2 lb patties. 1 inch thick. Press the center  to 1/2 inch. This will allow it to shrink even. 
Season patties with salt and pepper and place in hot dry skillet. 
When burger releases and crusts up from skillet flip. Repeat until Medium Rare is achieved.
For cheese burger move all burgers to center and place cheese on top of all burgers getting cheese. Pour shot glass of water in skillet and cover with lid for 20 seconds or less. Cheese will melt almost instantly due to steam.
Let rest on paper towel.
Place on lightly toasted bun inside only. This will help prevent soggy bun. I actually toast them in the hot skillet prior burger cooking. Lettuce helps also.
Never press a burger under any circumstance.
Adding stuff inside the meat imo works poorly and is a gimmick. Seasoning with salt prior causes meat to get hard. Personally I like the topping prepared by themselves.
 
Onions: Dice two large onions and place in a non stick skillet with a couple tablespoons oil. Cook until light brown. When light brown add 1/2 can Coke and cook down until Coke is gone. If you like Caramelized Onions you will prepare them this way from now on. 
Tomatoes: If you wouldn't eat it raw don't put it on a burger.
Mushrooms: Bake in 400 degree oven for five minutes. Then finish in skillet with butter. The oven bake will roast mushrooms and they wont sweat in pan.

That bacon is so beautiful I would have to take it off and enjoy every bite just by itself.  Then I would put the rest of the beautiful sandwich together and squeeze it down with both hands, and chomp my way through it -- after I add the thick slice of raw onion.

Darn tootin'.  Sometimes the best burger is a simple meat, cheese onion and ketchup.  Around here that is a common way at cookouts.  And Lawry's Seasoned Salt is seen very often instead of regular salt.

As far as "knife" (and fork) burgers go, "hamburger steak" dinners are still on many menus around here, still popular, at least with the older folks, I guess.  Usually about ten ounces, often "smothered" with onions or mushrooms or both, maybe gravy. 
 
Is that just in the rural Midwest, or elsewhere too?

Mar, thanks!  That is indeed a great alternative idea.  Since I got COPD six years ago, walking is not my best thing, and I used to go for miles, easily.  Now I'm pretty good on the flats, but hills and stairs are tough. We will be looking at these closely!

Seasoning with salt prior causes meat to get hard.
Last summer I conducted a little experiment and salted a burger 3 days before I grilled it.  When I finally ate it the texture and feel were somewhat sausage-like (not necessarily a bad thing) and it had good flavor.
