Quick Survey for Restaurant Staff (for a class)

I am looking to collect data from Restaurant staff members (waiters, waitresses, managers, etc.) for a Human Computer Interaction class project involving the design of restaurant sites.  I found this forum via Google and thought you might be able to help.  The survey is pretty short, so if you have a few minutes it would be appreciated.  Thanks in advance!

I just wanted to say that this message is not spam.  I am honestly working on a class project and need some feedback.  I am not even sure how a survey would constitute spam.

Here we have ump-thousand members and one or two who insist on flagging everything from a new poster as SPAM.  It is a system that is apparently condoned despite the fact we've seen it chase a way many new posters.
 
That said, I looked at your survey ... you seem to be selling nothing nad not asking for the respondee's email address.
